Gracefield Healthcare is a new service for 6 adults with Learning Disabilties, situated in a period property (St Domingo House) that has been renovated to a high standard
St Domingo House is a care home for adults with learning disabilities it provides a homely, family environment for 6 resident’s.
The staff team provide emotional and psychological support through all day-to-day aspects of the residents life.
All care staff receive training on a regular basis, to improve the quality and variety of care available to our residents, and carers work on a one-to-one basis with the resident
Staff are on waking duty 24 hours a day
The staff team also provide residents to identify and manage their behaviours by involving them in addressing the issues, formulating risk assessments and contributing to the development and evaluation of individualised care plans. It is also encouraged that family members and social workers are involved in this process.
We have a Multidisciplinary team including a Psychologist, Psychiatrist, Pharmacist, Social worker Psychiatric nurse
Independency is encouraged upon assessment, e.g. self medication and budgeting their own finances. Care plans are prepared with the residents to maximise their full potential enabling them to eventually pursue an independent way of living.
Regular meetings are held to encourage residents to voice their views on living within the Home, these are acted upon.
Residents are actively encouraged to take full part in all aspects of living from menu planning, shopping and budgeting and residents cook their own meals with staff support on a one-to-one basis if requested or required.
St Domingo house offers individual activity and educational programs which are formulated and agreed with the service user and then reviewed, as required, to ensure that individual abilities and needs are being identified and met.
Activities outside the home include the local college, adult training centres, vocational skills centres and our own diversional therapy team who work with small groups giving support to access the local community.
Within the Home we promote on a continual basis the development of social skills, independent living and personal responsibility
St Domingo House is a large Victorian property set over four floors with wheel chair access to the front and a large lawn garden to the rear.
The Home has six bedrooms, all are en-suite. All bedrooms are spacious, well-furnished and decorated to individuals taste.
Communal areas include the kitchen, dining room, laundry, lounge and a family room. The home has been refurbished to provide a comfortable and homely living environment. The Home also has two toilets and a bathroom.
The Home is within easy reach of local shops, parks, leisure centre, main bus route and only 10 minutes from the city centre
